Who is Seniors in Transition, LLC?
L. Kirsten Hartman, C.M.C.
Seniors in Transition is owned by L. Kirsten Hartman, C.M.C. Her path to gerontology included work as a social work intern, marketing associate in a retirement community, a legislative assistant, and a real estate broker. Kirsten has completed geriatric course work and holds a bachelor’s degree in Psychology. She is a member and former officer of ElderCare Network. Seniors in Transition is bonded and insured and is a proud member of the Better Business Bureau. Kirsten is a member of the National Association of Professional Geriatric Care Managers and is Certified by the National Academy of Certified Care Managers.
"I have always known that I love working with people. Years ago as a legislative aide, some of our constituents were asked to move out of their apartments due to the wave of condominium conversions. As I talked with these people, I realized in some cases how hard it was for them to make a change. Then I was at a conference with Ken Dychtwald, Ph.D. and became aware that issues in the aging population were only going to grow as the population aged. As a Realtor, what I liked most was listening to people’s needs and helping them solve their housing problems. Care management was a way to combine many of my skills and passions."

"I have been a Registered nurse for more than 35 years. It was while working in a hospital for physical rehabilitation that I realized how often patients and their families had no idea how to navigate the health care system and how to find the help and equipment they might need when they went home. This was the pivotal point when I decided to change the focus of my career and become a Certified Case Manager thru the Commission for Case Manager Certification. I have been in the caregiver role for several family members and continue with that role. This has given the ability to see and experience both sides of the spectrum. My goal is to help others to access quality care and services to make their 'golden years' be as bright as they can be."
